I understand that if I agree to a background check below I will be required to provide my Social Security Number.

I certify that the information contained in this application is correct to the best of my knowledge. I understand that to falsify information is grounds for refusing to hire me, or for discharge should I be hired.

I understand that by signing below I am consenting to a background check that I will receive by email to fill out and that my driving record will be viewed in order to be placed on company insurance if hired.

Upon completion of the background and driving record checks, if hired, I will be placed on a 60-day trial period and paid weekly as a contract laborer of which I will be required to pay my own taxes on payments received.

Upon completion of the 60-day period I will become an employee of the company and paid as one.

If I am not licensed by the State Fire Marshal License, Fire Shield Fire Protection, Inc. will pay for my apprentice license.

After completing my first 2 weeks I will have the opportunity to take the State Fire Marshal License test for portable fire extinguishers. Payment for the test will be deducted from my check and repaid to me upon passing.

If I do not pass, I may take it two other times of which I will be responsible for all associated testing costs (with the opportunity to receive 1 refund upon passing).

I authorize any person, organization or company listed on this application to furnish any and all information concerning my previous employment, education and qualifications for employment. I also authorize you to request and receive such information.

In consideration for my employment, I agree to abide by the rules and regulations of the company, which rules may be changed, withdrawn, added or interpreted at any time, at the company’s sole option and without prior notice to me.
